What Are Dark Eye Circles?
Dark eye circles are the darkening beneath the lower eyelids, caused by skin pigmentation, visible blood vessels through thin tissue, structural shadowing, or skin thinning over time. Often, a patient presents with multiple overlapping factors.
Three terms are frequently used interchangeably, but each refers to something distinct:
- Dark eye circles: Discolouration or shadow of the skin beneath the lower lid, vascular pooling, or structural shadows localised directly beneath the lower eyelid skin
- Eye bags: Physical protrusion or puffiness caused by herniated fat beneath the eye
- Tear trough: A hollow groove running from the inner corner of the eye down toward the cheek that creates shadow
What Causes Dark Eye Circles?
Understanding the contributing factors is what allows treatment to be matched to what is actually present.
Thin Under-Eye Skin and Vascular Visibility
The skin beneath the eye is among the thinnest on the body. As it thins with age or through genetic predisposition, blood vessels beneath become more visible. The breakdown products of haemoglobin, haemosiderin and biliverdin contribute to the bluish or brownish tones associated with vascular dark circles.
Structural Shadowing and Asian Anatomy
A less prominent midface bony projection, more common in Asian facial anatomy, means the under-eye area sits in relative shadow. This structural feature can make dark circles more pronounced independently of skin tone or pigmentation.
Tear Trough Formation
As the orbicularis retaining ligament and malar ligament loosen with age and cheek fat descends, a groove forms beneath the lower eyelid. This tear trough creates a shadow that is often mistaken for pigmentation but is structural in origin.
Collagen and Skin Thinning
Collagen production decreases at approximately 1 to 1.5% per year from the mid-twenties. As the skin loses thickness and elasticity, underlying structures become more visible and the texture of the under-eye skin may become crepey over time.
Genetic and Lifestyle Factors
Hereditary facial structure, fat distribution and skin tone all influence how the under-eye area appears. Sleep deprivation, dehydration, sun exposure, smoking and nasal congestion can exacerbate existing dark circles, though they are rarely their sole cause.
Most dark eye circles are multifactorial. A thorough assessment is the only reliable way to identify what is driving the appearance in any individual.
Who Gets Dark Eye Circles?
Dark eye circles affect men and women at any age, though certain factors make them more common or more pronounced.
- Older adults: Skin thins, collagen decreases and facial volume shifts with time, making vascular visibility and structural shadowing more pronounced.
- Darker and Asian skin tones: A higher baseline melanin level and, in many cases, a less prominent midface structure can make shadowing more apparent regardless of age.
- Those with a genetic predisposition: Deep tear troughs, hereditary pigmentation or particular facial fat distribution can mean dark circles appear from a relatively young age.
Seeking an assessment earlier rather than later tends to allow for a broader range of non-surgical options to be considered.
The Combination Approach for Mixed Dark Eye Circles
Most patients who present with dark eye circles have more than one contributing factor. A single removal treatment, however well chosen, may not fully address what is present. A combination laser-and-filler approach targeting both pigmentation and structural hollowness can address more of what is present than either treatment alone, because it works across multiple layers and causes simultaneously.
In practice, this might mean pairing a Q-switched or picosecond laser for the pigmentation and vascular components with under-eye fillers for hollowness and tear trough shadowing. Alternatively, skin boosters or Rejuran I combined with a collagen-stimulating treatment can address both texture and structure where shadowing is the dominant concern but skin quality is also a factor.
